How to Submit a Referral


Please send the following documents using a secure fax:

  • Completed LVAC referral form

  • Patient demographics and insurance information

  • Relevant clinical notes

  • Most recent access imaging (if available)

  • Current medication list

Self-Referral Option

If you’re a dialysis patient experiencing issues with your access—or if you’ve been told you may need a fistula, graft, or catheter—you don’t have to wait. At Lubbock Vascular Access Center, we accept self-referrals and can help you get the care you need quickly and safely.

Simply give us a call at (806) 792-2222, and our team will guide you through the next steps, including how to coordinate with your nephrologist or dialysis center.

What to Expect After Sending a Referral


1. We contact the patient to schedule their appointment.

2. We verify medical history and imaging needs as required.

3. We send a follow-up summary to your office after the procedure.
